It has been suggested that this not a real increase but is due to increased awareness and/or new classifications. If that is so then there must have been the same number of cases prior to the observed increase as there have been since. All efforts to identify these earlier cases in both the USA and the UK have failed. There are only two possible reasons for the absence of earlier cases.
