Picture a graph of the space-time continuum: Now, warp that graph as shown, and connect to parts together: The white is the space-time continuum, the blue is the outside of the wormhole, and the red is the inside of the wormhole. In order for this to happen, you would need a special type of matter (we will call it matter-x). Matter-x is theoretical, and we have no evidence of it's existence. Matter-x would need to push matter away from it instead of attracting matter towards it's self. If you had matter-x on both ends of the wormhole, and you could harness their energy to push out into space, then you might be able to bend space-time enough to connect the two points and form a wormhole.
